Lewy body dementia references
Lewy body dementia includes dementia with Lewy bodies and Parkinson’s disease dementia. The Parkinson’s questionnaire names dementia with Lewy bodies and directs the examiner to also complete the appropriate Mental Disorders questionnaire. Section 8A addresses mental effects attributed to Parkinson’s disease, Parkinsonism or its treatment.
The additional-document direction is for the examiner when applicable. It does not mean every claimant must obtain or purchase two private questionnaires, or that these diagnoses are interchangeable.
