Examiner requirements
Initial examinations: board-certified or board-eligible psychiatrist; licensed doctorate-level psychologist; or the supervised trainees specified on page 1. Review examinations may also include an LCSW under close supervision, or a nurse practitioner, clinical nurse specialist, or physician assistant under close supervision and clinically privileged by VHA for the required activities. The supervising psychiatrist or psychologist must meet the Veteran, confer on diagnosis and final assessment, and co-sign. Read the complete page 1 requirements. If the Veteran has a PTSD diagnosis, the PDF directs use of the Initial PTSD Questionnaire by a VHA staff or contract examiner instead of this questionnaire.
