Polyneuropathy

Polyneuropathy describes involvement of multiple peripheral nerves. It does not by itself identify the cause, nerve-fiber type or affected areas.

Keep the diagnosed cause and each affected area clear. The generic Peripheral Nerves questionnaire excludes diabetic sensory-motor neuropathy, which has its own questionnaire. Diabetes occurring alongside another neuropathy does not establish a diabetic cause.

Public Disability Benefits Questionnaire

Peripheral Nerves

The Peripheral Nerves questionnaire can record multiple diagnosed peripheral nerve conditions with separate symptoms, findings, nerves and sides. Use its diabetic exclusion when selecting the document.

On the original questionnaire: Page 1: title exclusion and fields 1A–1B; page 2: 2A and 3A; pages 3–5: examination findings; pages 6 and 8: nerve assessment introductions; page 13: diagnostic testing; page 14: functional impact.
