Chemotherapy-induced peripheral neuropathy

Some chemotherapy can cause peripheral nerve damage, called chemotherapy-induced peripheral neuropathy (CIPN). Effects may persist after treatment ends. Treatment history and examination findings help the clinician assess the cause and affected functions.

Symptoms after chemotherapy do not prove that treatment caused them. Keep other possible causes and coexisting conditions visible. The cancer itself and peripheral nerve effects are separate documentation questions.

Peripheral Nerves

The Peripheral Nerves questionnaire can describe chemotherapy-related peripheral nerve findings when its scope fits. The history, testing and functional-impact fields retain the treatment context and actual effects.

On the original questionnaire: Page 1: title exclusion and fields 1A–1B; page 2: 2A and 3A; pages 3–5: examination findings; pages 6 and 8: nerve assessment introductions; page 13: diagnostic testing; page 14: functional impact.
